The following forms are to be completed and returned to the University of Wyoming Sports Medicine Center no later than August 1, 2006. Please make sure the correct forms are signed by the student athlete and parent/guardian. All forms must be filled out and returned to the Sports Medicine Center in order for the student athlete to participate in any athletic team related activity.
