
Students wishing to receive accommodation services must register with UDSS. For more information on the application process, please call our office at (307) 766-6189, TTY: (307) 766-3073 to set up an appointment with a UDSS coordinator; or stop by our office in 330 Knight Hall, 8:00-5:00 PM, Monday through Friday, to pick up application materials. We also are happy to provide fact sheets about our services; please let us know if you need them in alternate format.
Checklist for Applying and Initiating UDSS Services (4 easy steps):
1. Complete the appropriate
UDSS application form (links for applications listed below).
One application is primarily for individuals with physical disabilities (e.g. mobility impairments, visual disabilities, hearing impairments, etc.).
The other is primarily for students with cognitive disabilities (e.g. learning disabilities, cognitive difficulties due to head injuries, seizure disorders, chronic fatigue syndrome, etc., psychological or psychiatric disorders, etc.)

2. Gather and send UDSS documentation of the condition(s) that maybe a disability for you.
The documentation needs to be current enough to explain how the condition(s) affects you now, especially in light of your anticipated need for assistance to accommodate your disability. UDSS has documentation guidelines to assist you locate the appropriate materials. Note these are guidelines, not strict requirements. The documentation you provide will be evaluated on an individual basis, along with the information you provide in your application for services. If you have questions about what is needed, please contact UDSS for assistance.

Your application and documentation will be reviewed as soon as possible
after receipt. You will be notified by phone or mail whether additional
information is needed and/or if you qualify for services. If you do qualify,
you’ll also be told which UDSS coordinator you’ll be working with.
4. Call our main office at (307) 766-6189, TTY: (307) 766-3073 and ask the
staff who answer to help you make an appointment to see your coordinator.
It's a good idea to bring your course syllabi or outlines to that
appointment. You and the coordinator may use that information to help
determine the details of services tailored to you.
