County Fair 4-H Entry Forms

Note!Tthe following forms are in Adobe Acrobat format. You will need an Acrobat reader to view and print them. You will also need to configure your WWW reader to use Acrobat as a helper application. Follow this link to obtain the free reader and for excellent instructions on configuring your WWW software.

In-structions for Sweetwater County 4-H Fair entries and using interactive entry forms:

1. Log onto the 4H4ME Website using your user name & password. If you do not have your user name and password, contact the UW Cooperative Extension Service Office at 307-352-6775 or 307-872-6310 to obtain them.
2. Check to verify all of your enrollment information is correct.
3. Access your project information and verify your 2008 projects.  Contact the UW Cooperative Extension Service Office at 307-352-6775 or 307-872-6310 with any problem.
4. Once your information is verified, you are ready to complete your 4-H Fair Entry Form.
5. All of Department H (NON-LIVE EXHIBITS), Divisions 16-73 (with the excep-tion of Division 30, Fabric & Fashion Entry Form  and Division 79 Showmanship), Divisions 76 & 78 (Livestock Judging & Herdsmanship),
   are on the TATIC EXHIBIT AND CONTEST ENTRY FORM. Begin by entering
   your demographic information and then your class entries.  Click on the
   down arrow of each interactive box and choose the Division, use the tab
   key to move to the next interactive box and choose the Class. Refer to the Sweetwater County 4-H Fairbook or the 2008 Member Exhibit Guide, if necessary.  It is important to complete the description of each item you
   are entering.  Use the mouse or the tab key to navigate through the form. Once the entry form is complete, use the “Print Form” button at the top of the page to print the form for your records; then click the “Submit by email” button to submit your entry information to the UW Cooperative Extension Service Office.
6. The Fashion Revue Entry Form is separate as well.
7. Department J, Youth Livestock Shows are broken down by species. 
   Click on the livestock project you are enrolled in and open the interactive form.  Enter all of your demographic information and then choose your classes.  Click on the down arrows of the interactive boxes to see the options.  Use the mouse or the tab key to navigate through the form.  Showmanship classes are at the bottom of each entry form.  Refer to the Sweetwater County 4-H Fairbook or the 2008 Member Exhibit Guide, if necessary. Once the entry form is complete, use the “Print Form” button at the top of the page to print the form for your records; then click the
   “Submit by email” button to submit your entry information to the UW Cooperative Extension Service Office.
8. If you simply cannot get your interactive form to work, please contact the
   UW Cooperative Extension Service Office at 307-352-6775 or
   307-872-6310 to complete your online entry.
