Hepatitis B Vaccine & Immunization Policies

Obtain Hepatitis B vaccine or other required immunizations at UW Student Health (Laramie), UW Family Practice (Casper, Cheyenne) or from your healthcare provider. Your provider must verify your immunizations on the Immunization Form linked below. If attaching copies of your medical records, complete the personal information on the front page of the Immunization Form. Do not fill out the immunization records yourself.

 

All Vaccines/ Immunization Documentation

  1. Hepatitis B Vaccine

    • Included documentation of the 3 steps on the Immunization Form above
    • $29 + $2 injection fee / per each of 3 doses (costs subject to change)
    • The series takes 6 months to complete.
    • The 1st & 2nd doses are administered 1 month apart and documentation of those first 2 doses must be in to the school by the deadlines set for your program.
    • The 3rd dose is administered 6 months from the 1st dose. Submit documentation to the school a the time when the 3rd dose is completed.

  2. Rubella & Rubeola Titre Results

    • Include documentation of the titre results on the Immunization Form above.
    • $10 each  (costs subject to change)
    • School of Nursing requires titres - not just MMRS - for admission to the nursing program (regardless of age). Titres consist of having blood drawn and tested for immunity (positive results).
    • If either titre is negative or equivocal, you will need to have another MMR and provide documentation. We recommend that you be re-titred 2 months after the MMR to see if you have gained immunity.

  3. Adult Td Booster

    • Include documentation of the Adult Td booster on the Immunization Form above
    • $23.45 + $2 injection fee  (costs subject to change)
    • The School of Nursing needs proof that this booster has been administered within the previous 10 years of your entrance to program, and should remain current through completion.

  4. 2-Step PPD (tuberculosis testing) or minimum of two years consecutive annual one-step PPD records with the most recent in year of admission

    • Include documentation of the PPD administration, reading and results on the Immunization Form above
    • $5 / each  (costs subject to change)
    • If 2-step PPD to be submitted:
      • PPD #1 is administered / read 48-72 hours later
      • PPD #2 administered 1 week (7 days) after #1 / read 48-72 hours later
    • If Annual PPD verification to be submitted:
      • The most current test should be dated within the year of admission; the second test, the year prior.
    • Anyone with a positive PPD history must have a chest x-ray completed.
    • You will be required to submit documentation of an annual one-step PPD each year after admission while in the program.

  5. MMR

    • Include documentation of the MMR on the Immunization Form above
    • $50 / each  (costs subject to change)

    • The MMR is required for University of Wyoming Admission of all students.

  6. Varicella (Chicken Pox)
    Students must provide one of the following to show history of Varicella:
    (Note:  this policy is under review and subject to final approval by faculty.)

    • Positive titre OR
    • Documentation of 1 dose of Varicella vaccine prior to age 12 OR

    • Documentation of 2 doses of Varicella vaccine if first dose given after age 12
