BRAND Program
Student Policies & Requirements

 

As part of your acceptance into the BRAND program, you will be expected to provide documentation and/or abide by the following School of Nursing policies and requirements. It is your responsibility to keep these requirements current throughout the completion of the program. Failure to do so will result in your being prohibited from attending clinical.

Start obtaining information requested according to dates as noted in links below. DO NOT wait until the last minute!

  2. Photo Request

    Please submit a picture of yourself (that actually looks like you!) no larger than 3" x 3" (can be smaller). If your picture is larger than the 3x3 dimensions, it will be cropped to fit our display. Print your name on the back of the picture and write your signature below your printed name. Submit the picture to the address above.

    The School will display photos with first names and last initial of the current students in a wall display so that faculty, staff and students can become familiar with new students. The display is a public accessible presentation of your picture/first name. Your submission of your picture with your signature on the reverse is your consent that we can use your picture in such a public presentation.

    • Background Check - complete by March 15
    • CPR - documentation due August 15
    • Malpractice Insurance - documentation due August 15
    • Release Forms - (when clicking into this link, you will need to scroll down to the "Release Forms" bullet and download the forms) due August 15
      • Clinical Policies Release Form
      • Educational Records Release Form
      • Photo Release Form
    • Immunization Form - completed form due August 15
  4. Requirements for Nursing Courses/Clinicals

    • Transportation - in place by the end of August

      Students admitted to the nursing major are expected to arrange their own transportation for clinical courses, because they need to travel to clinical agencies and home visit sites held locally and out of town.

    • Student Handbook

      You are responsible for knowing the content of the Composite Program Handbook for the year of your admission (BSN section). View and/or download the handbook by clicking the underlined link above.

    • Orientations - Attendance is Mandatory! - check back for scheduled date

      2. Safety and OSHA Orientation - To be held at the start of fall semester  Failure to successfully complete this requirement will result in your ineligibility to attend clinicals.
    • Heads-up on Drug Testing

      Drug screening may be required by some clinical sites.
