New Wireless Access Point Registration Form

If you are considering installing a new wireless access point on-campus IT requests that you provide the following information. In an effort to improve network security, wireless network access and wireless performance at UW, all new wireless access points need to be registered with IT.

Please complete all required fields before submitting. If information is missing or incorrect, you will be contacted by IT to obtain the correct information. Access points that are not properly reported via this form will be disconnected from the network.
