Your First ID Card

 

Spouses and Dependents of Students, UW Faculty, Staff, & Appointed Employees:
The first card for a spouse or dependent is $7.50. Proof of relationship may be required if last names differ and for dependents who are not natural or legally adopted children. Dependents are described as children (natural, legally adopted, legal guardian, or step children) under the age of 19 who have not graduated from high school. Replacement ID cards will cost $15.

Spouses and Dependents of Cooperating Agency Employees:
The first card for a spouse or dependent is $10.00. Proof of relationship may be required if last names differ and for dependents who are not natural or legally adopted children. Dependents are described as children (natural, legally adopted, legal guardian, or step children) under the age of 19 who have not graduated from high school.  Replacement ID cards will cost $15.
