Temporary Work Adjustment/Reasonable Accommodation Instructions
Reasonable Accommodation Determination
Contact the Human Resources Department to request an accommodation or work adjustment due to a disability or a serious health condition.
1. Accommodation Request – all employees must complete a Request for Accommodation form available through the Human Resources Department. Return the completed form to HR and they will review the request. http://uwadmnweb.uwyo.edu/hremployment/ADAReasonableAccommodationForm.doc.
2. Job Description – specific information is needed to respond to an accommodation request, therefore a job description or PDQ will be acquired by HR.
3. Health Care Provider Information – employees will be asked to have their health care provider review the job description or PDQ and specify any restrictions or modifications recommended due to the disability. The health care provider should diagnose and assess the particular disability as it relates to the requested accommodation and the job description/PDQ. The completed Request for Reasonable Accommodation, Section B. Medical should be returned to HR. Employees should not return the documentation from their health care provider to their department. It is the employee’s responsibility to see that their health care provider returns the required information to HR. Further, under some circumstances, a medical examination may be required by HR.
