The University of Wyoming is committed to ensuring equal employment opportunities to individuals with disabilities. First, the individual must provide documentation reflecting they are an individual with a qualifying disability under the Americans with Disabilities Act (ADA) or Section 504 of the Rehabilitation Act of 1973. This documentation needs to be submitted to the UW Human Resources Department (HR).  All costs and responsibility of documentation are borne by the individual with the disability. If the documentation is inadequate to make a determination as to qualifying disability and reasonable accommodation, HR may request additional information at the employee's expense.  Professionals within HR will determine if the individual has provided adequate documentation to be eligible for services.

1.      Documentation must be from a licensed professional trained in the specialty area. 

2.      Documentation may be investigated by HR as to its authenticity.

3.      Documentation must be on the Request for Reasonable Accommodation form with the licensed professional's original signature.

4.      Documents must be current and legible. 

Human Resources will make the determination of whether the condition is a protected/qualifying disability under ADA/504.
