Below please find a brief description of the UW's group long-term
disability plan provided to you through Guarantee Life Insurance Company (Jefferson Pilot
Financial). Questions concerning filing a disability claim should be directed to

What is the benefit amount?
- If you become disabled, this plan will be 66-2/3% of your monthly earnings to a maximum
benefit of $7,000 per month. The minimum monthly benefit is $100 or 10% of the Insured
Employees Monthly Benefit, whichever is greater.
How long do I have to be disabled before benefits begin?
- Benefits begin after you have been disabled for 90 days. Employees with earned sick
leave will remain on the universitys payroll until the sick leave is exhausted, and
then begin their disability benefit. Employees on donated sick leave will remain on the
universitys payroll for the 90-day elimination period and then begin the disability
benefit.
What is the definition of a disability?
- Benefits will be based on the inability to perform your own occupation for 60 months.
After 60 months benefits will be paid based on your ability to perform another occupation
for which you are reasonably suited by education, training, and experience.
Is there a pre-existing exclusion?
- Yes, if you have been treated for, taken prescribed medication or have seen your
physician three months prior to your effective date, you must be on the plan 12 months
without becoming disabled due to that condition (continuity of coverage applies).
Are benefits paid for partial disabilities?
- Yes, as long as there is a 1% loss in income. This plan also allows you to earn up to
100% of your pre-disability income while partially disabled.
Do I need to be totally disabled during the elimination period?
- No, you may satisfy the elimination period with partial disability days, total
disability or a combination of both.
If I leave employment, can I convert this benefit?
- Yes, there is a conversion option available if the employee has been insured for twelve
consecutive months prior to termination.
Does this policy integrate with other benefits?
- Yes, if you collect benefits from Social Security, Workers Compensation and/or
Wyoming Retirement Disability due to this disability, Jefferson Pilot Financial will
reduce the benefit payable.
Who pays for this plan?
- The University of Wyoming pays 100% of the premium for all benefited employees.
Is the benefit taxable?
