To establish or update your credential file:

Students

Students may stop by room 222 of Knight Hall to establish a file or call (307) 766-2398 to request the materials. Questions and requests can also be emailed to UWCACS@uwyo.edu Your file will typically contain:

  • Registration Form
  • Credential Form and/or Resume
  • Transcript (Obtained by CACS at students' request)
  • Student Teaching Evaluation
  • Letters of Recommendation

Alumni

Alumni may also establish a file and it will include the same items as the student file.

Electronic Materials

The required materials may be downloaded below. If you wish to submit the Education Registrant Information Form and/or the Credential Form via email, please follow these steps:

  • Click on the link below to download the file
  • Enter the appropriate information
  • Save the file to a location on your computer
  • Attach this file to an email message and send it to UWCACS@uwyo.edu

Education Registrant Information Form - for our database maintenance purposes; includes a standing release form which must be signed before credentials can be mailed out. Word RTF
Credential Form - outlines your educational/work experience; also includes your personal statement. A detailed resume can be used in place of the Credential Form, if desired. Word RTF
Letter of Reference Form- must be completed by three individuals, so you must print out at least three copies! This form must accompany "confidential" letters.  Word RTF
Wyoming Certified Application - Used to apply for jobs with Wyoming School Districts.  Word PDF

To save one of the above documents:

  1. Click on the link.
  2. Depending on your browser setup, one of the following will occur:
    • Microsoft Word (or a word processor) starts and loads the document. You may modify and save the document to the hard drive or to a floppy disk.
    • A dialog box pops up, asking whether you want to save or open the file. You may open it (and follow the instructions directly above) or you may save it directly to the hard drive or a floppy disk.
  3. If you choose to edit the files using Microsoft Word, please note that the documents are formatted with tables. Click in the appropriate cell and edit the contents of the cell. 

To send your credential file:

The Center for Advising & Career Services offers services to help you send your credentials to school districts. You need to have a file with us in order to use our services.

You may make your request by phone, fax, mail or email: UWCACS@uwyo.edu. CACS will do one of the following, based on what you request us to do:

  • Mail the credential to the district. CACS will process your request within 2 working days of receipt.
    The cost is $5.00.
  • Fax the credential to the district. CACS will process your request within 4 working hours of receipt.
    The cost is $10.00.

CACS will bill those students/alumni with a good credit rating.
