Here is the sample syllabus statement sent to us that the Associate Director of the Student Educational Opportunity Office and in charge of the University Disability Support Servicies has included in other materials our office shares with faculty and that you should include in your course syllabi:
“If you have a physical, learning, or psychological disability and require accommodations, please let me know as soon as possible. You will need to register with, and provide documentation of your disability to, University Disability Support Services (UDSS) in SEO, room 330 Knight Hall, 766-6189, TTY: 766-3073.”
Chris Primus adds this additional explanation for your information:
"There are many different ways to word such a statement. Others I've seen place some kind of deadline by which the student needs to make their needs known. I've discouraged including specific deadlines because we can't deny services based on an arbitrary deadline. The student does need to make a timely request in light of the accommodations they need, but it's not uncommon for students to, say, take the first exam with the class and then request accommodations from that point forward. If the documentation supports the request, we still need to provide it even though it wasn't initiated at the beginning of the course. At the same time, retroactive accommodations are neither reasonable, nor required."
