Subject: Course Action Request forms
On the A&S web page, under faculty and staff, are directions for submitting course requests (new courses, modifications, etc.) AND requests for USP designations and non-western certification.

Please note that when Course Action Request forms are submitted we need the original and 10 copies.  I know that the Course Action Request form itself calls for 9 copies.  But we need the extra copy because we keep a file copy in the Dean's Office.  On our web page, it clearly states that A&S needs 10 copies.
