Graduation/Title Form  This form puts you on the list for graduation.  Please type the appropriate information then either:
1) e-mail the file to your major professor and ask him/her to print the form out and sign it before forwarding it to your department head and Graduate School, or
2) print the form yourself and have your major professor and department head sign the form before it is submitted to the Graduate School.

Change of Status
Use the Change of Status form to change any one or a combination of adviser, major, minor, option/concentration, college, and/or graduate status.  You will be notified by e-mail or phone when the change has been made.  The Graduate School staff will then forward the form to the Office of the Registrar for you.
