Completion of Requirements Instructions

This form must be signed by all appropriate persons and returned to the Graduate School before a graduate degree can be granted.  It is the candidate’s responsibility to have this form completed correctly by the deadline date.  Check with the department or the Graduate School for the deadline date.

Please type the information at the top of the form (name, social security number, major, option, degree sought, and plan). 

COMMITTEE:  This form should be presented to the candidate’s graduate committee chair before the final comprehensive examination.  At the conclusion of the examination, the committee chairperson will have each member of the committee sign the form and vote to confirm or deny the granting of the degree.  It is possible for a committee member to abstain from voting, but a signature is still required and the committee member should mark the abstain box.  Once all the signatures have been collected, the committee chair should return the form to the degree candidate to complete the process.

FEES:  The Cashier’s Office will collect the appropriate fees and will sign and date this form.  Save the receipt provided by the Cashier and take it to the degree check appointment.

DEGREE CHECK:  A degree check must be completed during the semester in which the candidate graduates to certify the candidate’s current registration and completed coursework.  Each candidate should contact the appropriate Degree Analyst in the Office of the Registrar to set up a degree check appointment by calling (307) 766-3951.  The fees must be paid before completing the degree check appointment and the receipt must be shown to the Degree Analyst to verify the diploma fee has been paid.  However, it is not necessary to defend the thesis or dissertation before completing the degree check.

FORMAT CHECK:  The Graduate School will review the thesis or dissertation format after the final defense has been held and final corrections have been made.  Students writing a thesis or dissertation should follow the Thesis and Dissertation Format Guide available in the Graduate School.  The format check must be done before the thesis or dissertation is duplicated. 

SUBMIT FINAL MATERIALS:  The Completion of Requirements form must be submitted by the deadline date.  The Format Check and submission of a thesis or dissertation are not required of Plan B Master’s or Educational Specialists.

  • The Graduate School will collect the two archival quality copies of the thesis or dissertation after a final review for format and appropriate paper quality. 
  • The Graduate School will also ensure that the Survey of Earned Doctorates is filed appropriately as well as the microfilming agreement form for ProQuest Information and Learning.  These two steps are required only for PhD and EdD candidates.
  • A copy of this form will be given to the student at the time it is submitted to the Graduate School.  The department will be notified by email once the form has been received.

DEGREE CONFERRAL:  After the deadline date, the Dean of the Graduate School will review all material submitted with the Completion of Requirements Form.  If all requirements have been completed, the Dean will sign to confer the degree and a copy of the form will be sent to the Registrar for the candidate’s permanent record.  A copy will also be sent to the department for the departmental records.

The Commencement Program for the graduation ceremonies will be printed based on the information provided on the Graduation/Title Forms submitted to the Graduate School by mid-term of the final semester.

It is important to note that until this form is completed and filed in the Graduate School, the degree cannot be awarded.  It is the candidate’s responsibility to ensure the Completion of Requirements form is in the Graduate School (109 Knight Hall) by the published deadline.
